bought 8 spheres for this quest and only received 2 souls from spheres even though sphere was used throughout battle all 8 times...

Rod does not give a 100% chance to get the Soul for each killing blow. Been discussed before.

If you wish for description to be changed you're more than welcome to suggest it at suggestions forums here:
However by your logic every single quest , drop or any small thing that isn't 100% chance is misleading while they're not. It isn't correct saying it is misleading because by definition it would be misleading if it would mention one thing while it would be otherwise.
